Job Description: Social Worker (Choose Help services) is a critical member of our ministry by serving men to assist them with their individual needs to overcome their current circumstance. In Choose Help Services, Ambassadors build relationships with our clients, helping them see that they can change their circumstances. Once a client chooses help, a Social Worker designs a personalized service plan to end his/her homelessness. Choose Help Services is where ending homelessness begins for our clients.

You will also perform the following:

Essential Duties & Responsibilities:

 Leads weekly devotions

 De-escalates crises

 Utilizes triage process for mental health and substance abuse

 Completes safety screenings for clients fleeing domestic violence situations

 Assesses for suicidality and homicidally and recommends appropriate measures for individual and community safety.

 Provides best service recommendations based upon needs assessment results

 Researches and builds relationships with community partners and organizations to meet client needs

 Communicates with client’s external service providers on client’s status and progress

 Provides resource referrals

 Provides clinical guidance to team

 Leads life skills groups

 Performs other duties as assigned

Education: Eligible for Licensed Master Social Worker (LMSW) and practice under the supervision of an LCSW with supervision credentialing, Licensed Master Social Worker (LMSW) and practice under supervision of a Licensed Clinical Social Worker (LCSW) with supervision credentialing, or Licensed Clinical Social Worker (LCSW)
