Case Manager, Part-time
Company: Trinity Community Ministries Position: Case Manager, Part-time Location: Atlanta, GA Posted: 08/30/18 End Date: 09/29/18 Job Description: This position is responsible for providing a broad range of paraprofessional social services to assigned clients and delivering results based upon established goals and objectives. Job Duties: Contributes to improving program development for Trinity House – Big Bethel and Trinity Living Programs. Conducts comprehensive assessments for mental health and/or substance abuse that encourages full client involvement and leads to the development of individualized service plans with measurable goals and objectives throughout Trinity House service delivery area. Ensures clients receive education and treatment services through a continuum of care to achieve abstinence – based recovery. Represents TCM to the public through speaking at internal and external events. Provide direction and/or oversight of group/individual therapy sessions. Maintains documentation of client interaction via TCM’s outcome measurement data-base. Coordinates, prepare, and maintain required charting and documentation in a timely, thorough, and accurate manner. Adheres to all client confidentiality requirements and standards. Coordinates and/or facilitate individual and group counseling activities to address quality of life issues. Develop relationships with representatives in other agencies to support individuals attaining services such as additional mental health care, housing, psychiatric medication, medical resources, financial assistance, legal advocacy, etc. Participates in meetings, training, and development opportunities as determined appropriate by supervisor. Serves a Team Leader for a group of Resident Managers; problem solving, scheduling, etc. Monitors physical security around the facility, and activates appropriate emergency alarm systems.
Summons police or medical personnel as appropriate. Performs CPR and/or first aid if required. Other duties as assigned.
Education and Experience: A bachelor’s degree in Clinical Social Work or related field is preferred, or a minimum of two (2) years mental health and/or substance abuse case management experience accompanied by a high school diploma. Experience working with homeless population(s) is helpful.
